If you would like to request and consent to receive service via email, please do one of the following:

• Complete and submit the Request and Consent for Email Service form online
• Download a copy of the Email Service Request Form and submit it via email by clicking the Submit button or by attaching it to an email and sending it to email.service@soah.texas.gov
