The mission of the State Office of Administrative Hearings is to conduct fair, prompt, and efficient hearings and alternative dispute resolution proceedings and to provide fair, logical, and timely decisions.

General Comments and Inquiries

We welcome your questions, assessment of our services, and suggestions for improvements. If you have a general inquiry or comment about the services provided by our office, please contact SOAH's customer service representative, whose contact information is below.  We will acknowledge all inquiries and comments received by e-mail within one business day.  All others will be acknowledged within five business days.  You may use the SOAH Inquiry, Comment, and Complaint FormPDF(a 79 KB download file; the PDF requires Adobe Reader, which is a free download External web site) to submit a comment or inquiry.  Send your completed form to the customer service representative.

Formal Complaints

If you are dissatisfied with any service we provide and wish to file a complaint, you must do so in writing. A telephone call is not sufficient to initiate a complaint review and investigation.  We will accept a letter or other written communication stating your complaint, but if you wish to use a form, you may use the SOAH Inquiry, Comment, or Complaint Form.  You may also contact our customer service representative to request a form.

You should state clearly the nature of your complaint, including sufficient information to describe the misconduct that you believe occurred or the problem that you experienced. Once we receive your complaint, a written acknowledgment of receipt will be sent to you within five business days, including notice whether an investigation of the matter is warranted. The time to conclude a matter depends upon the nature of the complaint and its complexity.  Some may be resolved in 30 days or less; others may require substantially more time.  You will receive a written report of the resolution of your complaint.
